Resources · Continuous Support Support that doesn't stop when the paperwork ends

Real care is ongoing. We don't drop in and disappear. We stay genuinely involved, reviewing, refining, and realigning your support as your needs and goals evolve over time.

Reviews that actually happen

Progress reports, goal check ins, and plan reviews aren't paperwork we file away. They're conversations we have with you, your family, and your support coordinator, so your plan keeps up with your life.

When support needs may change

  • Experiencing illness, surgery, or hospital stays
  • Changes in informal support networks
  • Changes in behavioural support needs
  • Changes in level of function
  • Emergency situations

Our tools for continuous quality

  • Monthly progress reports sent to families, guardians, and support coordinators, summarising routines, goals, incidents, health, and social progress, and highlighting areas needing attention or celebration
  • Supervisor and manager checklists giving weekly oversight of documentation, routines, incidents, and behaviour changes, to spot early signs of fatigue, disengagement, or risk and ensure team accountability
  • Participant Goal Monitoring Plan: quarterly goal review and progress mapping, with structured, achievable goals tied to each NDIS plan, used to update care routines, community access and team focus, and to support plan review requests when needed

For families & support coordinators

  • A dedicated point of contact for each participant
  • Access to house supervisors and service managers
  • Scheduled updates, not just emergency calls
  • Real documentation that makes your job easier

If something needs to change

If a support arrangement stops working, EDC will update rosters, adjust routines, respond to behaviour changes with behaviour support plan (BSP) input, propose trial periods, and advocate for plan reviews when goals are being missed.
