Beyond our service list, here are the specific strengths that make transitions, monitoring and planning genuinely work.
Coordinating with discharge teams for medication, equipment, and care continuity.
Respectful handover of care plans, behaviour supports, and routines.
Reintegration support, behaviour planning, and social services.
Building confidence and readiness for this significant life change.
Seamlessly moving into long term housing and support arrangements.
EDC uses a structured, layered monitoring system:
Through partnerships with real estate agencies and SDA housing providers nationally, EDC can typically organise suitable housing within 2–3 weeks. This extends to emergency accommodation for participants facing urgent situations. See current vacancies.
Every support plan is built through real conversations with the participant and their support network. Cultural, sensory, emotional, and communication needs are all considered, and plans are reviewed as goals and circumstances change.
EDC works closely with allied health professionals, NDIS support coordinators, hospital teams, and families. Regular structured meetings, streamlined reporting, and open communication channels make collaboration efficient and effective.
